Private/Small Group Lessons Contract

 

This contract is between __________________________________ and Apex Lacrosse Coaching services.

 

I understand that I am entering into an agreement with Apex Lacrosse to provide private lessons for   Me      My child(ren)  Circle one      Signature ______________________________

 

• I understand that the cost of lessons is as follows:

Private Lessons for one student: 

• $160 Total / Four one hour lessons

• $320 Total / Eight one hour lessons

• $320 Total / Four two hour lessons

• $640 Total / Eight two hour lessons

• $40 Total / One trial one hour lesson

Semi-Private two student lessons

• $100 Total Per Student / Four one hour lessons

• $200 Total Per Student / Eight one hour lessons

• $200 Total Per Student / Four two hour lessons

• $400 Total Per Student / Eight two hour lessons

Small Group lessons for three or more students

• $80 Total Per Student  / Four one hour lessons

• $160 Total Per Student / Eight one hour lessons

• $160 Total Per Student / Four two hour lessons

• $320 Total Per Student / Eight two hour lessons

 

• I understand that I must pay at or before the final scheduled lesson, payment being given to the coach in the form of cash or check.

• I understand that if I or my child is in a group with two or more students and one of the group does not come to the lesson I can either pay the correct rate above or postpone the lesson.

• I understand that I can not sign up for or schedule any lessons until the past lesson package has been paid off, with exception to multiple packages being bought at the same time.

• I understand that if I or my child do not pay on the last lesson they owe the money and accruing interest until the total amount has been paid off. The compound interest is 10% for each day the amount exists. Contact using the links in the website to organize sending a late payment.

• I have read, understood, and agreed to Apex Lacrosse’s cancellation policy. 

Cancellation Policy:

Apex Lacrosse reserves the right to cancel or postpone lessons with at least 24 hours of notice in the event that the coach is sick or absent and a suitable replacement is not available. Customers have the right to postpone their appointment for up to two weeks by email or phone, with at least 24 hours of notice. If the postponement takes place within 24 hours of the scheduled appointment Apex Lacrosse may charge half of the price of the lesson to compensate the coach. If you would like to cancel your package you must first pay for all preceding unpaid lessons and a $22 cancellation fee.
